This survey provides us the opportunity to identify how well our patients are doing back in their communities and homes. These results also give us a method to observe the durability of our rehabilitation services and compare that with a national model. In the graph below, St. Luke's outcomes are compared with other rehabilitation facilities that use Med Tel Outcomes to collect post-discharge data.

Patients reported an average Functional Independence Measure (FIM) of 112.1, which shows better mobility and functioning of the patient, 90 days after discharge. A FIM score describes daily self care such as eating, dressing and toileting as well as mobility skills such as moving by walking or wheelchair and communicating their needs.
